Título : Estado oxidante-antioxidante en eritrocitos de pacientes con Enfermedad Pulmonar Obstructiva Crónica. “Efecto de contaminantes de la Atmósfera Material Partículado 2.5”

Resumen : Chronic obstructive pulmonary disease (COPD) is diagnosed by pulmonary function tests (spirometry) at different stages (0, I, II, III, or IV). COPD, causes an imbalance, metabolic called oxidative stress, which causes structural and functional changes in virtually all tissues and cells including erythrocytes, where it has a decreased ability to transport and diffuse oxygen to the tissues because the oxygenation capacity of hemoglobin is affected by the change of redox potential oxidizes to the ferric state (methemoglobin), which leads to tissue hypoxia. The redox state of hemoglobin depends on the efficiency of antioxidant system, intrinsic to the erythrocyte, which requires the use of efficient ways to generate ATP (without the respiratory chain) and a suitable reducing power represented by the presence of NADPH and reduced glutathione to maintain the integrity of the membrane. Patients with COPD are considered susceptible population to air pollution which is made up of gases and particulate matter (PM), the latter causing further damage to pulmonary patients who are affected when the material particles of a diameter less than 2.5 microns, can establish contact with the alveolar surface and alveolar capillary barrier through the translocation and may therefore cause systemic effects. This study evaluated the efficiency in the adaptation of erythrocytes from patients with COPD (GOLD I, II, III and IV), when they are subjected to in vitro challenge with PM to this effect were evaluated parameters of oxidative damage in erythrocyte membranes of patients with COPD, and the determination of enzymes involved in the process and reducing the generation of metabolites. With these results we conclude that it is possible that erythrocytes of patients with COPD, are injured by the progression of the disease, and that were more likely to increase the damage with the presence of particulate pollutants, compared to erythrocytes healthy volunteers. This is one of the first projects to demonstrate the differences in the responses of healthy populations and susceptible to pollution.
Descripción : La enfermedad pulmonar obstructiva crónica (EPOC) es diagnosticada mediante pruebas de función respiratoria (espirometrías), en diferentes estadios (0, I, II, III, ó IV). La EPOC, cursa con un desequilibrio, metabólico denominado estrés oxidativo, lo que origina modificaciones estructurales y funcionales en prácticamente todos los tejidos y células incluyendo a los eritrocitos, en donde se presenta una disminución en la capacidad para transportar y difundir el oxígeno hacia los tejidos, debido a que la capacidad de oxigenación de la hemoglobina se ve afectada por su cambio de potencial redox oxidándose al estado férrico (metahemoglobina), lo que conduce a la hipoxia tisular. El estado redox de la hemoglobina depende de la eficiencia del sistema antioxidante, intrínseco del eritrocito, que requiere de la utilización de vías eficientes para generar ATP (sin la cadena respiratoria), así como de un poder reductor adecuado representado por la presencia de NADPH y glutatión reducido, para mantener la integridad de su membrana. Los pacientes con EPOC son considerados población susceptible a la contaminación atmosférica la que se encuentra constituida por gases y material partículado (PM), este último es causante de un daño mayor en enfermos pulmonares, quienes son afectados cuando el material partículado, logra establecer contacto con la superficie alveolar y atravesar la barrera alveolo capilar por translocación, pudiendo en consecuencia causar efectos a nivel sistémico. En este estudio se evaluó la eficiencia en la adaptación de los eritrocitos de pacientes con EPOC (GOLD I, II, III y IV), cuando estos son sometidos a un reto in vitro con PM, para tal efecto fueron evaluados parámetros de daño oxidativo en membranas de eritrocitos de pacientes con EPOC, así como la determinación de enzimas relacionadas con el proceso y con la generación de metabólitos reductores. Con los resultados obtenidos concluimos que es posible que los eritrocitos de pacientes con EPOC, se encuentren lesionados por la progresión de la enfermedad, además de que mostraron una mayor probabilidad de aumentar el daño con la presencia de partículas contaminantes, en comparación con los eritrocitos de voluntarios sanos
