Resumen : | Objectives: The emergency physician is faced with critical patients who can not express the pain and sedation requiring mechanical ventilation, management is essential for the negative effects that this causes, the main objective of this study is to determine what are the most widely used analgesics in the emergency department is associated with sedation in mechanically ventilated patients in the emergency department HGR number 25 and determines whether the analgesic administered to the patient management inhibits the presence of pain. Material and Methods: Retrospective observational study. We captured two months patients on mechanical ventilation, moderate and mild sedation with midazolam, was applied to evaluate pain scale BPS to identify the level of pain was recorded in the data sheet given the analgesic management and descriptive statistical analysis was performed . Results: 56 patients were analyzed. The mean age was 60.45 years. The most common reason for intubation was sepsis, followed by respiratory problems. Pain intensity ranged from 4 to 11 points with an average of 7 (moderate intensity). Painkillers were the most used NSAIDs. The clonixinate lysine was the most common. 25% of the sample was not medicated. Conclusions: The most widely used analgesics to manage pain in critically ill patients are NSAIDs. The management administered does not inhibit the presence of pain. |
Descripción : | Objetivos: El urgenciologo se enfrenta a pacientes críticos que no pueden expresar el dolor por requerir ventilación mecánica y sedación, su manejo es esencial, por los efectos negativos que este ocasiona; El objetivo principal de este estudio es, determinar cuáles son los analgésicos más utilizados en el servicio de urgencias del HGR número 25 asociado a la sedación en paciente con ventilación mecánica y determinar si el manejo analgésico administrado al paciente inhibe la presencia de dolor. Material y Métodos: Estudio observacional descriptivo. Se capturaron durante dos meses pacientes bajo ventilación mecánica y sedación leve-moderada con midazolam, se aplicó la escala de valoración del dolor BPS para identificar el nivel de dolor; se registró en la hoja de datos el manejo analgésico otorgado y se realizo análisis estadístico descriptivo. Resultados: Se analizaron 56 pacientes. La edad media fue de 60.45 años. El motivo de intubación más frecuente fue sepsis, seguido de problemas respiratorios. La intensidad del dolor vario de 4 hasta 11 puntos con una media de 7 (intensidad moderada). Los analgésicos más utilizados fueron los AINEs. El clonixinato de lisina fue el más frecuente. 25% de la muestra no fue medicada. Conclusiones: Los analgésicos más utilizados al manejar el dolor en el paciente crítico son los AINEs.
