Título : EFECTO TERAPÉUTICO DE LA ACUPUNTURA SOBRE LAS MANIFESTACIONES CLÍNICAS Y LA CONCENTRACIÓN SÉRICA DE IgE EN PACIENTES CON RINITIS ALÉRGICA

Keywords: allergic rhinitis, IgE, Acupuncture. General Objective: Study the effect of acupuncture on the severity of symptoms of allergic rhinitis patients treated with acupuncture. Assess the effect of acupuncture on serum IgE in patients with allergic rhinitis. Methodology: We evaluated 15 patients (n = 15) with previously diagnosed allergic rhinitis by ENT (ear, nose, throat) specialist, underwent blood sampling per patient (5 ml) at baseline and at the end of the acupuncture treatment to assess serum IgE by ELISA technique. Evaluations were performed: questionnaire on the severity of symptoms in allergic rhinitis (CSRA) at the beginning and end of treatment, as well as the questionnaire on the severity of nasal symptoms and nasal not applied weekly (SSN and SNN). There were 10 acupuncture sessions (2 times per week) for 20 minutes each using the points: Shangxing (DM23) in the direction perpendicular at the coronal plane, Yintang (EXTRA) placed transversely directed toward the nose, Yingxiang (IG20) directed perpendicular to bilateral above; Hegu (IG4), Quchi (IG11), Zusanli (E36) and Sanyinjiao (B6) placed at a perpendicular angle to the skin and all at a depth of 10 to 20 mm approximately, manipulating them in tone up by rotation every 10 min. Results: We observed in the sample of 15 patients with allergic rhinitis clinical improvement in the thirteen symptoms studied through the questionnaire on the severity of symptoms in allergic rhinitis (CSSRA) and found that the decrease in 4 severity was statistically significant (p < 0,005). Each of the symptoms showed a downward trend which means that the presence of the end of treatment are lower: in the sneezing was noted an improvement of 63.58%, runny nose improved by 62.84%, nasal congestion improved by 59.03% nasal itching improved by 63.15%, postnasal discharge improved by 62.84%, ocular symptoms improved by 67.82% throat symptoms improved by 69.36%, chronic cough improved by 61.53%, symptoms improved in one ear 65.03%, headache improved by 66.66% and improved mental function by 75%. The concentrations of IgE in serum of patients studied (n = 15) with a record before and after acupuncture treatment taking into account that the normal concentration of IgE in serum is> 5UI/ml and <150UI/ml is found: at the beginning it was observed that 33.33% (5) of patients, the rank was increased, the 6.66% (1) was found below the normal range and 60% (9) were within normal values IgE. Final concentration was found in 33.33% (5) of the patients were above the normal amount of IgE, the 13.33% (2) were below the normal figure and 53.33% (8) were within normal values of IgE. Conclusions: The application of acupuncture at points Shangxing (DM23), Yintang (EXTRA), Yingxiang (IG20), Hegu (IG4), Quchi (IG11), Zusanli (E 36), Sanyinjiao (B6), reduces the intensity of symptoms in patients with allergic rhinitis (p = <0.005 for 13 variables symptomatological). In studying the IgE variable behavior was observed in the concentration, therefore, concluded that these changes were not apparently dependent on the acupuncture treatment.

Evaluar el efecto acupuntural sobre la concentración sérica de IgE en pacientes con rinitis alérgica. Metodología: Se evaluaron 15 pacientes (n=15) con rinitis alérgica previamente diagnosticada por especialista en Otorrinolaringología, se les realizó una toma de muestra sanguínea por paciente (5ml) basal y al termino del tratamiento acupuntural para evaluar la concentración sérica de IgE por medio de la técnica de ELISA. Se realizaron las evaluaciones: cuestionario de la severidad de los síntomas en rinitis alérgica (CSRA) al inicio y final del tratamiento, así como, el cuestionario de la severidad de los síntomas nasales y no nasales aplicados semanalmente (SSN y SNN). Se realizaron 10 sesiones acupunturales (2 veces por semana) de 20 minutos cada una utilizando los puntos: SHANGXING (DM23) en dirección perpendicular antero-posterior, YINTANG (EXTRA) colocado transversalmente dirigido hacia la nariz, YINGXIANG (IG20) perpendicular bilateral dirigido hacia arriba ; HEGU (IG4), QUCHI (IG11), ZUSANLI (E36) y SANYINJIAO (B6) colocados en ángulo perpendicular a la piel y todos a una profundidad de 10 a 20 mm aproximadamente; manipulándolos en tonificación por rotación cada 10 min. Resultados: se observó en la muestra de 15 pacientes con rinitis alérgica que mejoraron clínicamente en los trece síntomas estudiados mediante el cuestionario de la severidad de los síntomas en rinitis alérgica (CSSRA), obteniéndose que dicha 2 disminución en la severidad es estadísticamente significativa (p<0.005). Cada uno de los síntomas presentó una tendencia a la baja lo que significa que la presencia de estos al final del tratamiento son menores: en el estornudo se noto una mejoría del 63.58%; escurrimiento nasal mejoró en un 62.84%; congestión nasal mejoró en un 59.03%; prurito nasal mejoró en un 63.15%; descarga retronasal mejoro en un 62.84%; síntomas oculares mejoró en un 67.82%; síntomas de garganta mejoró en un 69.36% ; tos crónica mejoró en un 61.53%; síntomas de oído mejoró en un 65.03% (Gráfico 11); dolor de cabeza mejoró en un 66.66% y las funciones mentales mejoraron en un 75%. Se cuantificaron las concentraciones de IgE en suero de los pacientes en estudio (n=15) con un registro antes y después del tratamiento con acupuntura tomando en cuenta que la concentración normal de IgE en suero es >5UI/ml y <150UI/ml se encontró: al inicio se observó que el 33.33% (5) de los pacientes el rango se encontró aumentado; el 6.66% (1) se encontró por abajo del rango normal y el 60% (9) se encontraron dentro de los valores normales de IgE. En la concentración final se encontró que en el 33.33% (5) de los pacientes estaban por arriba de la cifra normal de IgE; el 13.33% (2) se encontraron por debajo de la cifra normal y 53.33% (8) se encontraron dentro de los valores normales de IgE. Conclusiones: La aplicación de acupuntura en los puntos Shangxing (DM23), Yintang (EXTRA), Yingxiang (IG20), Hegu (IG4), Quchi (IG11), Zusanli (E 36), Sanyinjiao (B6), disminuye la intensidad de los síntomas en pacientes con rinitis alérgica (p=<0.005 para las 13 variables sintomatológicas). Al estudiar la IgE se observó un comportamiento variable en la concentración, por lo tanto, se concluye que estos cambios no fueron aparentemente dependientes del tratamiento acupuntural.
